Additional Information of Eligibility:
Eligible applicants will include all 56 States and territories as follows: in the 24 States with existing AoA Evidence-Based Disease Prevention cooperative agreements, eligibility is limited to the recipient State agency of those existing cooperative agreements in the 32 States and territories that do not have existing AoA Evidence-Based Disease Prevention cooperative agreements, only State units on aging and State health departments are eligible to apply.

Regardless of which agency applies, both the State unit on aging and State health department must be full partners in the development and implementation of the proposed program.

The State Medicaid Agency must also be involved in the development and implementation of the program.

Only one application will be permitted for each State or territory.

To be eligible for support under this announcement, a CDSMP must meet the following criteria: 1) The intervention has been tested through randomized controlled trials and has been shown to be: i.

effective at improving and/or maintaining the health status of older people; and, ii.

suitable for deployment through community-based human services organizations and involve non-clinical workers and/or volunteers in the delivery of the intervention; 2) The research results have been published in a peer-reviewed scientific journal; and 3) The intervention has been translated into practice and is ready for broad national distribution through community-based human services organizations.

CDSMPs that meet the above criteria are listed in Attachment D.

Applicants may also propose other chronic disease self-management programs not listed within Attachment D, but the program must meet all the criteria for an evidence-based CDSMP program described above.

NOTE: States may support either one or two different CDSMP models under this announcement but no more than three.
