Medicare Rural Hospital Flexibility Program Evaluation Cooperative Agreement
This notice solicits applications for the Medicare Rural Hospital Flexibility Program Evaluation Cooperative Agreement. The Health Resources and Services Administration (HRSA), Federal Office of Rural Health Policy (FORHP) is accepting applications for fiscal year (FY) 2018 Medicare Rural Hospital
Flexibility Program Evaluation Cooperative Agreement. The purpose of this program is to provide data and conduct analysis to inform improvement efforts in Critical Access Hospitals (CAHs). CAHs will benefit by having resources related to financial and operational improvement, quality of care reporting and improvement, emergency medical service (EMS) integration and population health activities. CAHs will have access to analyses of relevant data and evidence-based best practices to improve their outcomes. CAH focused improvement efforts will also inform other CAHs and state Flex programs.

Additional Information of Eligibility:
Eligible applicants include domestic public or private, non-profit or for-profit organizations. Institutions of higher education, faith-based and community-based organizations, tribes, and tribal organizations are eligible to apply. Applicants may be a single entity or a consortium.
