In keeping with this mission, the Centers for Medicare & Medicaid Services has recently established the Money Follows the Person Rebalancing Grant Demonstration Program wherein they aim to assist the states to balance their long-term care systems and help Medicaid enrollees transition from institutions to communities.
The Money Follows the Person (MFP) Rebalancing Demonstration Program possesses the ability to provide critical tools that can be utilized to address the gaps in the availability of community services for individuals with disabilities.
Furthermore, the programs gives the states the opportunity to serve more individuals in home and community-based settings and this can help them execute the integration mandate of the American Disability Act.
