The NIH consistently works towards the achievement of its mission, which is "to seek fundamental knowledge about the nature and behavior of living systems and the application of that knowledge to enhance health, lengthen life, and reduce the burdens of illness and disability."
In keeping with this mission, the National Institutes of Health has recently established the Behavioral Interventions to Address Multiple Chronic Health Conditions in Primary Care Program wherein it intends to utilize common conceptual models in creating independent interventions that will contribute to the modification of health behaviors and the improvement of health outcomes in patients diagnosed with co-morbid chronic diseases and health conditions.
Co-morbid chronic diseases several chronic diseases that are affecting one patient alone. This condition requires multi-disease care management approach, and the behavioral interventions that are involved must have a significant impact in improving patients' health outcomes in individuals with three or more chronic health diseases.
