Multiple Approaches to Support Young Breast Cancer Survivors and Metastatic Breast Cancer Patients

The proposed program’s purpose is to increase the availability of supportive services for young breast cancer survivors (YBCS), young women with metastatic breast cancer (mBC), and their caregivers and families by supporting organizations and entities that serve this population.

will address strategies relevant to the target audience.

These include:
the establishment of a network of survivors and caregivers to facilitate policy, system, and environmental change interventions that increases access to lifestyle programs, clinical preventive services, and cancer care among survivors; development and implementation of innovative technological educational opportunities for health care providers on topics relevant to the delivery of appropriate treatment and care of YBCS and mBC; and utilization of patient navigation and community health worker program strategies to reach underserved populations to enhance their access to and utilization of YBCS services and programs.

To this end, activities funded by this NOFO, are intended to support organizations in closing the disparity gap needed to support young breast cancer survivors.

Specifically, organizations will implement strategies that aim to reduce disparities observed in survival and quality of life due to race, ethnicity, or other social determinants of health.

This NOFO will also support organizations in providing support services/programs to YBCS and mBC; and provide resources for health care providers in caring for all YBCS and mBC regardless of race, ethnicity, age, or sexual orientation during the period of performance of 09/30/2019 - 09/22/202 4.
