Recovery Act - Communities Putting Prevention to Work

The purpose of the supplement is to provide State Health Departments with resources to collect BRFSS baseline and follow up data from communities funded under Communities Putting Prevention to Work (CPPW), CDC-RFA-DP09-912ARRA0 9. The amount of funding for individual states will be determined through
