Additional Information of Eligibility:
a) Applicant is an existing health center that has had an application approved for grant support in FY 2011 under the Health Center Program (sections 330(e), (g), (h), and (i)).

b) The proposed project(s) within the application will support alteration/renovation costs.

c) Application request for funding as presented on Standard SF-424C is less than or equal to $500,000.

d) Alteration/renovation activities associated with the project will not begin prior to the award date.

e) The project(s) will not be used to support space which will be rented to other entities for the purpose of generating revenue.

The project space must be operated by the grant recipient to support services consistent with section 330 of the Public Health Service Act.

f) Application proposes to support the costs of alteration/renovation of a facility that is within the health center’s current approved scope of project (i.e., listed on Form 5B).

g) The proposed project(s) cannot be a sub-recipient or contractor site.

h) The project(s) proposed within the application will not occur at a site that received construction-related funding (i.e., new construction or a/r/r project) through the Capital Improvement Program (CIP), Facility Investment Program (FIP) or FY 2011 Capital Development funding opportunities.
